The “2014 completed form sample” you added is not exactly the same as the Ankara German Embassy Form. It was made as an example according to the German Embassy in Istanbul. There are some additional additions and deficiencies in the Form at the German Embassy in Ankara.
In the new one, it is not the name of your Mother and Father but the surname, as in the previous one, the name and maiden name have been removed. There is only the last name part .. It seems unreasonable to me to write only the last name. I think we should write the name as well. Am I reading it wrong ???

1- “Date of entry into Germany?” I have a Visa Appointment on June 19th and I will have a wedding on August 10th if God permits, and I want to go with my wife on August 23rd. (Our official wedding was held) If I write to 23.08.2014?
2 – “Where will you maintain your permanent residence outside the Federal Republic of Germany?” Would it be a problem if I say yes to that? Do you mind if I say no? Can you explain?
3- “Will other members of your family come with you or after you?” Maybe in the future my parents or siblings will come with a tourist visa, insha'Allah. In such a situation, do I have to write this?
4- “What kind of place do you live in?” Which circle does this part say? or is it the other room my wife's home in Germany Does my room in the house I live in Turkey asks?
5- “How long do you want to stay in the Federal Republic of Germany?” here PERMANENT okay if i write
6- “How will you make a living?” My wife is working and what should I write here. When I get there and find a job, of course, we will make a living by working.
7- “Ich beantrage die Aufenthalterlaubnis für …………………………. I request that a residence permit be granted for Tage/Monat(e)/Jahr(e) ………………… day/month/year.” To this part PERMANENT can we write? How much time do they give at most?
